AT-HM List Checker

Is this item funded under the AT-HM scheme?

Wondering if a piece of equipment or a home modification is covered under the Support at Home AT-HM scheme? Use the tool below to search the official AT-HM list — type a product name, category, or common term and see how it maps to the funded items. This tool is designed for occupational therapists, physiotherapists, care coordinators, and aged care providers who need a quick reference when planning a client’s support

How to use this tool:

  1. Type a product or term in the search box — use the name you know it by (e.g. “commode”, “gopher”, “bath seat”).
  2. Check the match colour — green means your term matched an official item name directly; amber means it’s a close or common-name match worth verifying; no result means nothing matched, so try a broader term.
  3. Click the drop-down arrow — each card shows the official item name, its ISO code, its AT-HM category, and any conditional notes that apply.

What is the AT-HM scheme?

The Assistive Technology and Home Modifications (AT-HM) scheme is a component of Australia’s Support at Home program, which replaced the Home Care Package program in July 2026. Under the AT-HM scheme, eligible older Australians can access a set pool of funding specifically for small aids and equipment (assistive technology) and home modifications — things like walking frames, shower chairs, grab rails, and ramps — to help them stay safe and independent at home. AT-HM funding is separate from the general Support at Home budget and is spent from a defined list of approved items, each with its own funding category. For full details on eligibility and how to access the scheme, visit the Department of Health and Aged Care website.
